New Patients Forms
If you would prefer to save yourself the time of filling out the new patient forms when you arrive, please feel free to use the link below to fill them out online before your first appointment.
First Visit
For your first visit, plan to arrive a few minutes early with your patient paperwork already completed and submitted, or come 15–20 minutes early to fill out the paperwork in the office. Be sure to bring your insurance card and personal ID. Once the paperwork is submitted, the doctor will review it, and you’ll be taken back to the exam room at your appointment.
The doctor will conduct a comprehensive consultation and physical examination to determine the patient’s condition and develop a preliminary treatment plan. If further exams, such as x-ray or MRIs are required, the doctor will provide a referral to the patient.
If it is deemed appropriate treatment will be administered on the first visit! Recommendations for at-home care may also be given at this time.
